This privacy statement documents how J&K Insights Limited (website address JandKInsights.net) complies with European Union data protection requirements . It applies to the website. The Company has 2 members Kathleen and Jonathan who are both Directors. Please note, we do not have a booking system on the website, we have an automated email "Contact Form." The email Contact Form is accessed on the home page of the website - (click here to email,) and we have provided a business address (12 The Blossoms, Fulwood, Preston PR29RF) and contact phone numbers (shown on the home page of the website,) in the event you may be interested in making a booking or an initial request for a free consultation. J&K Insights Limited is fully committed to ensuring that your privacy is protected. Any information that you provide will only be used in agreement with this Privacy Statement.
J&K Insights Limited may change this policy from time to time by updating this page. Please check this page from time to time to make sure that you are happy with any changes. PRIVACY NOTICE - HOW YOUR INFORMATION IS USED

Requests received via "My Contact Form" are sent by automated email, to J&K Insights Ltd as a website enquiry. It also shows the senders name, email address, message, time/date sent, and IP address. For privacy reasons it is not accessible to staff at WebHealer and is not stored on any of their systems. After the sender's message it reads:-
It may not be disclosed to or used by anyone other than this addressee, nor may it be copied in any way. If received in error, please contact WebHealer on 03455570300, and then delete it from your system.
After we (J&K Insights Ltd) have responded to your enquiry it is deleted and not stored electronically.
Our Insurer requires that enquirers who may become clients complete a Consultation Form (Health Questionnaire), ensuring that client and therapist are kept safe. The goal is to try to make the therapy a client specific provision. It is a form of screening to make sure there are no contra-indications; to identify when not to proceed due to either a medical condition or medication. We also ask you to obtain approval from your GP/Healthcare professional when you have a pre-existing medical condition, take medication or feel unwell.


PRIVACY NOTICE - HOW YOUR INFORMATION IS USED

The information is used to be able to provide a service (therapeutic) that is specifically tailored to meet the client's requirements and to comply with insurance and code of conduct/ethics. For talking therapies that are remote (i.e. Zoom, WhatsApp, Skype, telephone), the therapist (J & K Insights Ltd) sends section 2 of the Consultation Form for the client (data subject) to sign and date. This is sent as an attachment using encrypted email and is password protected. The email address it is sent from is Kathleenfrancis1959@protonmail.com The client must return the signed/dated Consultation Form section 2 to this email address using password protection. Receipt of the signed/dated Section 2 Consultation Form authorises the therapist to complete the whole of the consultation form during the remote consultation with confidential input from the client. For non-remote, free consultations the client completes the whole of the Consultation Form. Please note J&K Insights Ltd takes no responsibility or liability for the client sending information back to an incorrect email address, the client takes full responsibility for this. The free remote consultation can proceed the same day upon receipt of the signed section 2 Consultation Form if there are no contra-indications etc and the client wants to continue to have therapy or to book a future appointment.


PRIVACY NOTICE - WHAT WE COLLECT

For remote therapy, receipt of the signed/dated section 2 Consultation Form gives consent to the therapist to complete the client's confidential information during the free consultation, avoiding the need for the enquirer ie potential client to send confidential sensitive information electronically.
For non-remote, free consultations the client will be asked to complete the whole of the Consultation Form as follows:

First Name, Surname, Date of Birth, Address, Email, Telephone number, Occupation, Referred by (if applicable,) Marital Status, Number and age of Children or Dependants, Doctor's name (your Doctor will not be contacted without your consent. Doctor's Address, Doctor's Telephone number
For non-remote therapy you will be asked to tick all items that are pertinent to you shown below: NON-REMOTE THERAPY IS NOT PROVIDED DURING GOVERNMENT RESTRICTIONS.
Do you suffer, or have you ever suffered from any of the following: (If so then please give details), Diabetes, Kidney problems, Epilepsy, Photosensitivity, Surgery or injury to back or joints, Sensitive skin, Allergic skin rashes, Allergies, Varicose veins, Deep vein thrombosis, Heart disease, High or low blood pressure, Cancer, Stroke, Asthma, Migranes. We also ask you to include on the Consultation Form any condition that you are suffering with.
If you are currently taking any prescribed or self prescribed drugs or remedies? In the next 12 hours, will any of the following apply:

Driving or operating heavy machinery, drinking alcohol, using a sunbed or exposing your skin to ultraviolet light.

For remote Hypnotherapy - upon receipt of signed/dated Consultation Form Section 2 the therapist will complete the Health Questionnaire during the free consultation obtaining information from the client; to do this remotely during the session the therapist will ask the client the following :
First Name, Surname, Date of Birth, Address, Email, Telephone number, Occupation, Referred by (if applicable,) Marital Status, Number and age of Children or Dependants, Doctor's name (your Doctor will not be contacted without your consent).Doctor's Address, Doctor's Telephone number

The Insurer, Policy Bee, requires us to keep the Consultation Form (Health Questionnaire) and Therapists Record Card for 6 years and then it is disposed of using a shredder.
The Insurer, BGI Insurance & Risk Management, requires us to keep the Consultation Form (Health Questionnaire) and Therapists Record Card for 6 years, then it is disposed of using a shredder.
This information will not be disclosed or used for marketing purposes, it is only used to conform to the policies and procedures of our insurers.
We will store the information you give us securely, in line with the Data Protection Act 1998 and the General Data Protection Regulation (GDPR). Please note: It is not stored electronically. It is stored in a locked cabinet in a locked office, only the practitioner has access to the cabinet.


HOW WE USE YOUR INFORMATION - WE NEVER ENGAGE IN TELESALES OR DIRECT MARKETING

We use this information to make sure we are conforming to our insurers policies and procedures and that the client and therapist are kept free from injury or harm. Ensuring we practice competently and ethically and conform to our Associations and Professional bodies code of Ethics, Policies/Procedures.

We may contact you about a specific booking or in response to an enquiry via our website. Where permission is obtained or a legitimate interest is shown at the time of receiving a therapy we will inform you about any special offers or workshops or mini classes.


SHARING INFORMATION

We will never sell or share your information with any other organisation for marketing purposes or any other purposes. We will only share your information:

If we are required to do so by law.
If it is directed by a Court.
If the benefits to a child or young person or vulnerable adult that will arise from sharing the information outweigh both the public and the individual's interest in keeping the information confidential.

We explain to you during the free consultation that everything you disclose to us is confidential unless you declare that you are going to harm yourself or someone else and disclose a safeguarding issue.


SECURITY

We are committed to ensuring that your information is secure. To prevent unauthorised access or disclosure, we have put in place managerial procedures to safeguard your information. We delete your information from our Website, it is not stored electronically. WebHealer does not store your information. We never leave the website without logging out. It is only accessed by a private password.
We use a password protected encrypted email address and ask the client to send their response using the same password protected email address.

It is the client's responsibility to make sure that any confidential sensitive information is sent to the correct email address. J&K Insights Ltd is not responsible or liable for any confidential and sensitive information that the client sends to the incorrect email address. The enquirer ie potential client must take full responsibility for sending their confidential and sensitive information to the correct password protected encrypted email address.
